To start the initial paperwork, please setup a new client portal and fill out the initial paperwork.

In order to initiate addiction/SUD outpatient program, an assessment must be completed by an LADC. If you have had an assessment done at another treatment center/clinic, please contact that treatment center/clinic and have them send it to PRO. If you still need an assessment completed, please select the "Client Portal" button above to fill out the initial intake paperwork. A member of the PRO staff will be in contact with you once this is completed to schedule your assessment/orientation appointment. 

Once an assessment is received by another treatment center, the orientation appointment can be scheduled. During orientation, an LADC will go through all of the necessary paperwork with the client and answer any questions the client has about treatment. The client will receive a treatment folder and be scheduled for their next appointment. 

If you are interested in initiating individual mental health therapy services, please select the "Client Portal" button above and fill out the initial paperwork. A member of the PRO staff will contact you to schedule an intake/assessment appointment. 

We have immediate openings for therapy (virtual-only). Most commercial and state plan insurances are accepted (private pay as well).
